What is Medical Coding?

Medical coding is the technique of analyzing the various components of patients' charts and identifying diagnoses (abstraction), along with assigning alphanumeric codes based on established guidelines.                ​
Steps to Coding:

Read a patient's chart (subjective, objective, assessment, and plan).

Identify and locate the chronic and acute conditions.
Picture
Review guidelines (if any) pertaining to those conditions.
Picture
Assign an alphanumeric code based on the guidelines and proper documentation.
